The insurance policy defines the promise. The claim is the moment it's tested. And yet, for most insurers, consistent policy-based adjudication is simply not the operational reality.


Portfolios built over decades, through new product launches, acquisitions, and broker scheme additions, have produced hundreds or thousands of policy variants, each with slightly different terms, exclusions, and conditions.
No claims handler can hold all of it. No RAG system or LLM can be trusted to get it right consistently. Hallucinations are inherent, outputs too vague to act on. No auditable trace from claim facts to policy clause to decision, which is exactly what regulators and dispute processes demand.
So the default became: pay below a threshold without checking, or route complex cases to subject matter experts. Slow, inconsistent, expensive, hard to manage and impossible to scale.
